TVFC/ASN Vaccines in Inventory have expired

Provider reports expired doses as Waste (Vaccine Loss) in VAOS. Be sure to select the accurate Reason for Waste from the following options:• G81 = Expired; did not notify HSR or LHD 90 days before expiration• G81 = Expired; notified HSR or LHD 90 days before expirationOnce waste is entered in VAOS, Provider downloads their Vaccine Loss Report. A shipping return label will be sent to the Provider’s PVC within 1 week.

After 1 week, did the Provider receive their Shipping Return Label?

Provider emails corresponding Vaccine Loss Report to Responsible Entity and TXVaccineOrders@dshs.texas.gov.In your email, specify what email address you would like the shipping return label sent to.

TXVaccineOrders team will request the label be sent/resent to specified email address.Provider should receive return label within 5 business days

Pack spoiled vaccines into box(es), include VLR with contents, and seal the box. No cold packs are needed

Attach provided shipping return label to outside of container, then give box to UPS driver when they next visit your site

Provider can enter new Vaccine Request to replace spoiled vaccines in VAOS
