Diagnosis: IgG Lambda MM (R-ISS II, CRAB+)

FISH:No high-risk cytogenetics,

PET-CT: L3 & scapula lesions

Started VRD regimen

Switched to Carfilzomib-based regimen (stable disease)

ASCT performed → Lenalidomide maintenance

Lost to follow-up

Relapse: L3 lesion with soft tissue extension

M spike 2.7 g/dL → Daratumumab-based therapy

Relapsed again after Daratumumab

Progression: enlarging L3 lesion, soft tissue component

Started Teclistamab (No CRS/ICANS)

Persistent symptoms, MRI: diffuse infiltration, progression
