Step 1 – Criteria for starting HFNC wean:
Patient Criteria – SpO2 ≥ 90%, RR ≤ 24/min, No Respiratory distress
Device Criteria – HFNC FiO2 ≤ 50%, flow > 20L/min
Stable for 6–12 hours
Decrease flow by 10L/min every 20 min.
Patient criteria met as per Step 1?
Increase flow to prior setting
Patient criteria met?
SpO2 < 90%
Increase FiO2 up to 100%
Check criteria
Screen every 6-12 hours
No
Increased respiratory effort or distress
Repeat Step1 in 6-12 hours
Escalate respiratory support
Step three- Decrease FIO 2 By 10% every 20 minutes until FIO2 Less than or equal to 50%.
Increase Fio 2 until Spo 2 greater than or equal to 90%. Repeat Step 3 in 6 to 12 hours.
Increase flow
